In this conclusion to the Joe Pitt casebook series, it begins with Joe homeless and living in the sewers of New York. It ends with the majority of the vampire power structure wiped out and Joe riding off into the sunset with his lady love and a doomsday device to ensure that he is not bothered.

One night while hanging out at his favorite bar, Joe comes into contact with another victim of the Vyrus who appears to be wigged out on some kind of new drug called Anathema. Since the Vyrus cancels out every other stimulant, this gets Joe's attention.

Joe Pitt is one of those men who could start a fight in an empty room. Not that he needs to, there's a whole city full of vampires who have an axe to grind with him. After the events of the last book, Joe has been exiled the South Bronx. He's doing his best to keep a low profile and eke out enough of a living to keep him in blood, bullets and smokes. It's not easy. He's there on sufferance -- despite the tolerance and interest of local boss Esperanza -- and he's down to his last three bullets.

He struggles with his past as he works odd jobs and tries to stay out of the grasp of the Mafia-like vampire factions that rule Manhattan.

He's been infected by a Vyrus that slows aging, imparts phenomenal strength and sensory abilities, and survives by feeding off its host's blood -- which forces its host to go out and drink more blood so the Vyrus can survive. And now there's a growing drug problem in the Vampyre community, some really bad stuff that makes users go crazy -- not easy to manage for those infected with the Vyrus, which is solicitous of its hosts and cleans drugs and alcohol out of their systems almost as fast as they go in.

He's been infected by a Vyrus that slows aging, imparts phenomenal strength and sensory abilities, enables almost instantaneous healing, and survives by feeding off its host's blood -- which forces its host to go out and drink more blood so the Vyrus can have plenty of sustenance. There's a whole Vampyre subculture in New York City, loosely gathered into Clans or collectives -- a hidden world of power and struggle unsuspected by ordinary human beings who live their lives in daylight. In this secret world, Joe's something of an outsider.
